ECHOCARDIOGRAPIC MEASUREMENT OF LEFT VENTRICULAR MASS IN NIGHTTIME DIPPERS AND NON-DIPPERS DURING 24HR AMBULATORY BLOOD PRESSURE MONITORING

Abstract:
PURPOSE: To determine the differences in echocardiographic (ECHO) left ventricular mass (LVM) in patients who are either nighttime dippers or non-dippers based on their 24hr ambulatory blood pressure monitoring (ABPM) recording.
METHODS: ECHOfile and 24hrBPfile, the ECHO and ABPM modules of CARDIOfile, our cardiology database were searched for all patients (P) who had both an ABPM and an ECHO within 1 year of each other. For simplicity only systolic blood pressure (SBP) was used in this analysis. A night-time dipper was defined as a >10% difference in mean systolic blood pressure between day-time (0700-2200hrs) average and night-time (2200-0700hrs) average SBP. There were 2,070 P used in this analysis. An unpaired Students t test was used to determine statistical significance between means.
RESULTS: Of the 2,070 P, 1,693 (81.8%) were nighttime non-dippers and 377 (18.2%) were nighttime dippers. The major results are seen in Table 1.
CONCLUSIONS: Non-dippers are significantly older and have increased left ventricular mass. There is no significant difference in overall average 24hr SBP indicating that the increased LV mass is due to the failure of SBP to fall during the nighttime period.
